CHICAGO SKY ONLINE SURVEY SWEEPSTAKES RULES & REGULATIONS

(1)     No purchase necessary. (2) Completion of the Survey is not required for participation in the Sweepstakes. (3) Eligibility: Sweepstakes open to residents of the state of Illinois residing within a 75 mile radius of Allstate Arena.   Must be 18 years of age or older as of June 20, 2016. Employees and immediate families (spouse, parents, siblings, and children) of Chicago Sky, the Women’s National Basketball Association and its Member Teams, WNBA Enterprises, LLC and each of their respective parents, subsidiaries, affiliates, directors, owners, employees and agents (collectively, the “WNBA Entities”) are not eligible to enter.

How to enter: (a) completely fill out and submit the online Survey, including Sweepstakes entry form at https://nba.turnkeysurveyor.com/se.ashx?s=705E3F0F3FCD351A; or (b) fill out and submit Alternate Sweepstakes Entry Form at http://sky.wnba.com/dish-sweep Upon completion, you will be automatically entered in the Sweepstakes; Entries are limited to one entry per email address. Entries are limited to one entry per person per email address regardless of method of entry. (5) Contest Term: June 21, 2016 through July 1, 2016. Entries must be received by July 1, 2016. (6) Winner Selection: Winner will be chosen from a computer generated random sample of all entrants and will be notified via telephone or email by July 24, 2016. In the event the potential winner does not respond to prize notification within 30 days of attempted notification or declines the prize, another winner will be randomly chosen by computer from the remaining eligible entrants. Chances of winning depend on the number of eligible entries received. (7) Prize: One (1) official Women’s National Basketball Association basketball autographed by players on the 2016 Chicago Sky will be awarded as the prize for this sweepstakes. The prize is non-transferable to any other person, including relatives or friends. Winner is solely responsible for all applicable federal, state and local taxes and any expenses associated with the game, unless otherwise indicated. No substitution, cash equivalent or transfer of prize permitted except at Chicago Sky’s option for a prize of comparable or greater value. Approximate retail value (“ARV”) of prize: $70.00. ARV reflects unautographed merchandise. ARV of hand-signed merchandise subject to fluctuation based on the featured WNBA players and current market value at the time prize is awarded. (8) Winner’s List: The winner’s name can be obtained by writing to Chicago Sky Online Survey, C/O WNBA Research Department, 645 5th Avenue, 14th Floor, New York New York, 10022. (9) Miscellaneous: By entering, entrant agrees to follow the Official Rules and hold harmless and indemnify Chicago Sky, and/or the WNBA Entities against any and all liability, damages or causes of action (however named or described), with respect to or arising out of either (a) entrant’s participation in the Sweepstakes; or (b) the receipt or use of the prizes awarded in the Sweepstakes. (10) In consideration for being awarded the prize, winner hereby agrees and consents(where legal), without further notification, authorization, compensation or remuneration of any kind, to the use of winner’s name and/or likeness in any and all advertising, promotions and other publicity conducted by Chicago Sky and/or the WNBA Entities.
